PASSPORT INFORMATION

Acceptance Hours: 9:00 AM - 11:00 AM & 1:00 PM - 2:30 PM Monday through Thursday

APPOINTMENTS ARE REQUIRED, CALL 541-367-5128 TO SCHEDULE

ROUTINE PROCESSING TIME: 6 TO 8 WEEKS AVERAGE*
EXPEDITED PROCESSING TIME: 2 TO 3 WEEKS AVERAGE*
*It may take up to 2 weeks from the day your apply until your application is "processing." 
If you need your passport in less than this time frame, please tell your Passport Acceptance Agent.

DO NOT SIGN THE APPLICATION UNTIL REQUESTED!

All applicants must be present. Both parents must accompany children aged 15 and younger. Children aged 16 & 17 must be accompanied by one parent.

PLEASE BRING THE FOLLOWING ITEMS TO YOUR APPOINTMENT (no photocopies accepted):

1. Completed DS-11 Passport Application (DO NOT SIGN APPLICATION)
2. Proof of citizenship
   • ​State Certified Birth Certification with parent(s) name(s)
   • Naturalization Certificate or
   • Certificate of Citizenship or
   • Report of Birth Abroad
3. Proof of identity
   • Valid Driver's License or
   • State-issued Identification Card
     • If you have an out-of-state license or state-issued ID card, please bring supporting identification to support your Oregon residency. 
     • In the case of minor applicants, parental ID will be used.
4. Fees
   • 1st Fee for passport book, passport card, and any expedite/overnight fees MUST be CHECK OR MONEY ORDER payable to the U.S. Department of State
     • Passport for all forms of travel (including air): Passport book
       • Adult 16+: $130.00 (valid 10 years)
       • Child 15 and under: $100.00 (valid 5 years)
     • Passport for limited forms of travel (ex. land crossings from U.S.): Passport card
       • Adult 16+: $30.00 (valid 10 years)
       • Child 15 and under: $15.00 (valid 5 years)
     • If needed within 6-week average delivery (expedite fee): $60.00
     • Overnight fee to guarantee 6-week average delivery: $19.53 (not available for passport cards)
   • 2nd Fee for processing ($35.00) payable by cash, check, money order, or credit card payable to the City of Sweet Home
   • 3rd Fee for photos ($15.00) IF requested, payable by cash, check, money order, or credit card payable to the City of Sweet Home
     • The 2nd and 3rd fees may be combined into a single payment method.
