What is needed for your appointment:
- Completed DS11 Form. (Do NOT sign the application until your appointment)
- All applicants must be present at the appointment. Both parents must accompany children aged 15 and under. One parent must accompany children 16 and 17 years of age.
- Proof of Citizenship (one of the following items)
- State Certified Birth Certificate
- Naturalization Certificate
- Certificate of Citizenship
- Report of Birth Abroad
- Proof of Identity
- Valid Driver’s License
- State Issued Identification Card
If you have an out of state license or a state issued identification card, please bring supporting identification. (Parents Identification will be used for minor)
- Passport Fee. Must be a check or money order made payable to US Department of State
